All new cosmetic patients, or established patients seeking new treatment, are required to have a consultation prior to receiving treatment. Our consultation fee is $75.
Note: consultation fee is due at the time of scheduling your consultation and can be applied to your treatment fees. Consultation fee is non-refundable if appointment is cancelled or patient “no shows” for the scheduled appointment.
Additional fees or deposits may be charged for repeat late cancellations and “no shows” for scheduled appointments.
Children under the age of 16 are discouraged from accompanying adults who are receiving treatment.
All clients interested in laser procedures or chemical peels are required to have a consultation with the physician before any procedures are performed. Unfortunately, laser procedures and chemical peels are not appropriate treatments for all skin types or certain skin conditions. Our staff will perform an in-depth consultation at the time of your consultation to determine if you are a candidate for the desired treatment. Consultation fee is $75 and can be applied to treatment plan.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
